iv extracted all the files from the zip to a new folder on my desktop but im not 2 sure what im meant to be right clicking on

On the "hospital.exe" file - right click on it then select properties, compatability tab. Then select "Run this game in compatability mode for" Windows 98/ Windows ME.

I don't have high hopes for you though running TH on Vista..see my comment above.

iv done everythin u said and wen i doubble click on it to load the game the screen goes black and it says language setup failed.... so does this mean that it aint gonna wrk on the pc??

This is probably because it isn't in the original installation folder ...
