I have a Boot Disk, made from BootDisk.com for Windows 95, but I am not 100% sure how to use it, it doesn't come with any reading material.
Any additional help would be appreciated, thank you in advance.

If you remember, the CD needs to be set to boot in the BIOS. If you need help accessing the BIOS let me know. The instllation CD that Ubuntu comes on is a live/bootable CD. It has the drivers for the drive so all you need do is put the installation CD in the drive and boot the PC.
To use the boot disk just put the disk in drive A: then boot making sure drive A: is set to boot first. When you come to the C:> prompt type D: (assuming the CD worked and the drive was assigned D

If you can not access the CD doing this then most likely the drive is bad. Before giving up on the drive check and make sure the cables are connected properly.
